While doing this medication reconciliation assignment I did have some concerns while speaking with this client. One of the the concerns that I had is that the client is hypotensive and both of the thyroid hormone medications the client is on can cause unusual bleeding. If the client starts to bleeding significantly, they should seek medical care immediately because the client could face life-threatening effects and their blood pressure could drop even more. Another concern that I had was that levothyroxine has an adverse effect of anxiety and client has anxiety and is being treated with medication. I would monitor to see if the client has a period of more anxiety when taking both of these medications. The other safety concern would be that some of these medications cause dizziness, so this puts the client at a risk for falls. I would want to educate the client to be cautious and monitor how these medications affect them before doing any vigorous activity or operating a car. These concerns stemmed from the client’s current medical history and how these medications can cause the client to experience other adverse effects that they might not have expected. The client was aware on why they were on 3 out of the 4 medications. However, the client was not sure why they were still on metformin. The client was aware why they were put on metformin 16 years ago, but is not sure why they are still on it today. The client has not been diagnosed with diabetes and doesn’t experience any of those symptoms. If it is possible for this client to talk to their provider and ask why and maybe be taken off metformin. The client is on two medications that treat their hypothyroidism. When talking to the client, they seemed to know why they were on both and were very knowledgeable about hypothyroidism. The client stated that they have been diagnosed with hypothyroidism for quite a while and understand how these two medications will help to treat their hypothyroidism. The adverse effects that I talked about with this client was hypotension, unusual bleeding, upper respiratory tract infections, dizziness. The client was aware of some of these adverse effects but not all of them. The two that the client did not know about was the unusual bleeding and the upper respiratory tract infections. I would continue to educate the client on ways to watch out for these adverse effects and report if these adverse effects are happening frequently.
